  • I am a provider on a telehealth platform that accepts insurance called Nourish. It takes about 15 minutes to sign up and get an estimate of your costs - most people pay $0!

    Sign up HERE. When asked "How did you hear about us?" add → Referral, and type Gabriela Czapek.

    • Depending on your insurance plan, Nourish may require a co-pay per session.

    • If your insurance does not cover dietitian services you may need to pay out of pocket.‍ If this is your situation, I’d recommend going back to my homepage to explore the packages available. Or book directly through my HIPAA-compliant portal HERE.

    • Discounts are available to Extreme Fitness gym members. Message me for more info!
